We recognize and understand that a diagnosis of a life-limiting condition brings a difficult and emotional time. Here at Longfield we listen to you, answer your worries and questions, and support you and those around you with physical and emotional wellbeing.   

Getting support as a patient will enable you to be yourself, rather than being defined by your condition. You may struggle with things from time to time, and we can help you adapt, manage your life more effectively and improve your quality of life. We achieve this by providing a broad range of activities to help manage your condition, from diagnosis and onwards.

How we can help

Our Wellbeing Services are free of charge and accessible to adults living with a life-limiting illness as well as their family, loved ones and carers. To access these services you must registered with a GP in Gloucestershire and be referred. Our drop in sessions are our only sessions you can attend without being referred prior.

If you are unsure of your needs, get in touch and we can chat through the ways Longfield may be able to help you or your loved ones. We can talk over the phone or via video call to determine how best we can help, or if there is an alternative organisation that would better suit your needs. Our team is professional and friendly and will listen carefully to your needs before making any recommendations.

How to Refer

If you would like to access our Wellbeing Services you can either refer yourself or be referred by a loved one/carer a family member or a healthcare professional. If someone is making a referral on your behalf, they must have your consent to do so. We take referrals by phone, email or by filling out and submitting the online form below. If you are making a referral by email, please download and fill out our referral form and attach it to the email. If you would like to discuss anything prior to referral, please reach out to us by phone or email.
