Your Life, Your Death – Advance Care Planning Group

A group aimed at enabling you to discuss and explore end of life care planning. There will be a range of specialists providing sessions on topics such as Lasting Power of Attorney’s, writing a will, funeral planning, anticipatory grief, the psychological process and symptom management. There will be a light lunch served from 12:30-1:30pm.

This service is free of charge if you are living with a life-limiting illness or are a family member, loved one/carer or bereaved of an individual with a life-limiting illness. To access this service you, a family member, a loved one/carer or a healthcare professional can put in a referral and once the process is complete and you are accepted into our care, you can book these courses by getting in touch with the wellbeing centre.
