Your Life, Your Death – Advance Care Planning Group

A group aimed at enabling you to discuss and explore end of life care planning. There will be a range of specialists providing sessions on topics such as Lasting Power of Attorney’s, writing a will, funeral planning, anticipatory grief, the psychological process and symptom management.

This group is designed to help you think about what is important to you and how to make your wishes known to your loved ones. You will also be given the opportunity to build a Memory box over the 8 weeks, gathering and creating memories and reflections to share.

This service is free of charge if you are living with a life-limiting illness or are a family member, loved one/carer or bereaved of an individual with a life-limiting illness. To access this service you, a family member, a loved one/carer or a healthcare professional can put in a referral and once the process is complete and you are accepted into our care, you can book these courses by getting in touch with the wellbeing centre.
