Hospice at home
Considering how you want to be cared for is difficult and can be upsetting to discuss with your loved ones. When you plan-ahead and share what is important to you, and how you would like to be cared for, your loved ones will find comfort in knowing that they are carrying out your requests. It will also help you to stay in control, as your condition progresses.
We provide care within the comfort of your home based on your needs, which may include frequent visits during the day from one or two of our team and care overnight if you need it. We aim to support the management of symptoms such as pain, breathlessness, and nausea, and to give your family support. They may also benefit from the chance to have a rest, take a break, or spend some relaxed time with you. Our team of skilled and experienced health care assistants and registered nurses operate 365 days of the year for care for patients at the end of their life.
Our Hospice at Home is free of charge and accessible to individuals at end-of-life, with a life-limiting illness who are registered with a GP in Gloucestershire.
How to refer for Hospice at Home?
The Patient will need to have a completed CHC Fast Track Referral form which can be completed by a Healthcare Professional, such as a district nurse, GP, consultant or clinical nurse specialist.
Referral Criteria
- The patient is an adult with a diagnosis of an advanced, progressive life-limiting illness and a prognosis of three months or less.
- The patient and family / carers consent to a referral being made to our Hospice at Home service.
- The patient is registered with a Gloucestershire GP.
- The patient is on, or to be accepted onto, the community nursing team’s caseload.
