Referrals for Hospice at Home

If you would like to discuss a specific patient or make a referral please telephone the Hospice at Home Team on 01453 733704. We always welcome a conversation to establish if Longfield can provide the most appropriate care for your patient.

Working hours are Monday – Friday 9-4.30pm

In an emergency please telephone 01453 733704, 7 days per week 7am-10pm. Please note this option is for professionals referring to Hospice at Home only.

Referrals for all other services

If you would like to make a referral for any of our services, excluding Hospice at Home, please call the Wellbeing Team on 01453 733706, or email wellbeing@longfield.org.uk.

Working hours are Monday – Friday 9-4.30pm

All our services are provided free-of-charge to those living with a life-limiting illness at the end of life and are accessible to adults registered with a GP in Gloucestershire, or who live in Gloucestershire.
